What is QCIPN?

The Queen’s Clinically Integrated Physician Network (QCIPN) is an integral part of  The Queen’s Health Systems that extends care beyond the traditional clinical setting. We provide patients with coordinated whole-person support to improve health, wellbeing, and access to care.  

Our interdisciplinary team includes social workers, nurses, navigators, pharmacists, and dietitians who work collaboratively with your care providers to ensure you receive the support you need. 

Team-Based Care Model

Free, Voluntary Programs and Services to Help Patients Get Well, Be Well, and Stay Well

While participation may be dependent on program availability and patient eligibility, the QCIPN Clinical Care Coordination Team offers free programs and services to patients who see a provider in QCIPN.  Programs include but are not limited to:

  • Addressing Medical or Psychosocial needs (financial, food, housing, transport assistance)
  • Advance Care Planning
  • Behavioral Health Assistance
  • Caregiver Support/Education
  • Classes to Help Manage Falls and Increase Activity Levels
  • Coordination of Care After a Hospitalization
  • Diabetes Management
  • Health Education
  • Hula Classes for Hypertension
  • Medication Management and Education
  • Nutrition Guidance
  • Smoking Cessation
