It is the Honolulu Coalition’s (alongside the other Hawaii coalitions) hope that this website will help providers and community partners meet the needs of Hawaii’s patients and caregivers during transitions of care by improving ease of access to Hawaii’s local healthcare resources.
The Department of Community Services, Elderly Affairs Division, has produced this resource directory to serve the community, service providers and our Kūpuna, with the goal of assisting our seniors with aging in their homes and communities for as long as possible.
Kōkua Mao has various resources on advance care planning (ACP) available in several languages. Resources on their website range from videos about how to discuss advance care planning with loved ones to the difference between ACP forms and downloadable form templates.

QCIPN members have access to over 200 ACP Decisions videos in 20 languages. These shared-decision making tools offer patients and families educational videos that accurately portray the medical conditions they face, explain their treatment options, and help them partner with their providers to plan for future medical care.
Through HHIE, you can access patient medical records from a variety of different health care systems across the Hawaiian Islands. HHIE’s Health eNet puts community health records, electronic referrals tracking and HIPAA-compliant, encrypted Direct Secure Messaging at subscribers’ fingertips.
TigerConnect allows you to communicate safely and securely with colleagues at the point of care from your smartphone, tablet, or desktop. Features include: dynamic electronic scheduling system, automatic shift handoffs, patient tracking, ADT notification, critical lab results, paging system, and answering services.
