Every Practice should have an assigned staff member to be the “Super User” to manage the patient Care Gaps and Attributions in Coreo.
Workflow:
For Well Visits
The “Super User” should be reviewing the patients’ care gaps in Coreo prior to their visit (few days in advance) to identify, notate, and prepare for the appointment. This will allow the physician to maximize their time and not miss the opportunity to close any and all care gaps at the time of the appointment.
For Sick Visits
While triaging, the Medical Assistants should be reviewing the patient’s chart to ensure all the care gaps are met. If care gaps are noted, the physician should be notified, so it can be taken care of at the time of visit (i.e., bundle visits, update vaccines, complete screeners, etc.).
